- HMO plans typically require that policyholders live in the same state that the company is based in due to the network coverage.
- PPO plans usually support out of state coverage.

If your company currently only has an HMO or in-state only plan, hiring an out-of-state employee or an employee moving could be a qualifying event for your company to add another plan with a wider network outside of renewal.
